The following constitutes a disposition of tangible personal property to my last will and testament or revocable trust:

Upon my death, I wish to have my United States Military Academy Class Ring donated to the Class Ring Memorial Program administered by the Association of Graduates of the United States Military Academy. I agree to allow the ring to be melted and its gold mixed with other gold, which will then be used to produce rings for the Cadet classes of the United States Military Academy receiving their class rings after my donation.

My United States Military Academy Class Ring should be sent to the Director of Class Support, Office of Alumni Support, Association of Graduates, 698 Mills Road, West Point, NY 10996.

In many states this should be filed with your Last Will and Testament or revocable trust, and mention of this document should be made in either of those documents.
